Waiver of Liability, Assumption of Risk, and Medical Authorization

1. Assumption of Risk

I understand that participation in basketball camps, clinics, and related activities conducted by 1UP involves inherent risks, including but not limited to physical injury, illness, falls, contact with other participants, equipment-related injuries, and other risks associated with athletic activities. I voluntarily choose to allow my child to participate and fully assume all such risks, known and unknown, even if arising from the negligence of the Company to the fullest extent permitted by law.

2. Release and Waiver of Liability

In consideration of being allowed to participate, I, on behalf of my minor child, hereby release, waive, discharge, and covenant not to sue 1UP, its owners, directors, officers, employees, coaches, volunteers, agents, sponsors, and facility owners from any and all claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained while participating in or attending any 1UP basketball camp or related activity.

3. Indemnification

I agree to indemnify and hold harmless 1UP from any loss, liability, damage, or cost, including attorney’s fees, that may incur due to my or my child’s participation in the camp or due to my or my child’s actions.

4. Medical Authorization

I certify that my child is physically fit and has no medical conditions that would prevent safe participation. In the event of an emergency, I authorize 1UP to obtain medical treatment for me/my child if I cannot be reached. I understand that I am financially responsible for any medical care or treatment provided.

5. Insurance

I understand that 1UP does not provide health or accident insurance for participants. I certify that I have adequate insurance coverage for my child or agree to bear all costs of injury or illness.

6. Code of Conduct

I understand that participants are expected to follow camp rules and instructions from coaches and staff. 1UP reserves the right to dismiss any participant for unsafe or inappropriate behavior, without refund.

7. Governing Law

This agreement shall be governed by and interpreted in accordance with the laws of the state in which the camp is held.

I HAVE READ THIS WAIVER AND RELEASE CAREFULLY, FULLY UNDERSTAND ITS TERMS, AND SIGN IT FREELY AND VOLUNTARILY. I UNDERSTAND THAT BY SIGNING THIS DOCUMENT, I AM GIVING UP SUBSTANTIAL LEGAL RIGHTS.
